Q. Who is Chart Access, Inc.?

A. Chart Access is a Houston based firm dedicated to medical record retrieval, with the primary focus on Attending Physician Statements for the insurance industry.  Chart Access is the APS division of Keais Records Service, a litigation support medical records company, established in 1975.

Q. Does Chart Access offer nationwide record retrieval?

A. Yes. Over 27 years in the record retrieval industry has allowed us to develop a system that efficiently and effectively accesses medical information across the United States, Mexico and Canada from our headquarters in Houston, Texas.

Q. How do I order medical records from Chart Access?

A. The best way to order records is to submit your request on-line through our secure, HIPAA compliant web site.  Simply complete a few questions regarding the patient, provider and ordering party.  Chart Access will do the rest.  Patient authorizations can be attached to the on-line order.

Q. Does the same processor handle a case from start to finish?

A. Yes. Each order will be handled by a trained team member assigned to your account, and every order will be handled by that team member through completion of the order.  You will have a name, email address and toll free phone and fax number to use in order to access your team member.

Q. How long are status notes held before they are purged?

A. Status notes are never purged and are permanently archived on our system.  Medical records are held in the system for a minimum of six months.

Q. Can the scope of records being requested be standardized so that redundant information does not have to be entered each time?

A. Yes.  The scope of records can be standardized in effort to reduce the amount of time it takes to enter an order, and eliminate the chance the requestor may enter incorrect information.

Q. How does Chart Access know who to contact if additional information is required on any case?

A. Chart Access’ on-line order form has three required fields of information:  user ID, password and email address.  ID and password identify which firm is ordering.  Email address will tell us who to contact with any questions.  This is all the information we need in order to handle your request.

Q. How does Chart Access charge for retrieved records?

A. Chart Access offers three pricing options: flat fee, fee for service plus facility charge and performance based fee plus facility charge. Our system is designed to offer our clients choices.

Q. Does Chart Access offer the use of a credit card in order to pay for records?

A. Yes.  We take advantage of the credit card for prepayments and medical records whenever possible. Chart Access keeps a database of every location ever visited which offers important details on repeat visits to providers and facilities. For example prepayment amounts, credit card acceptability, checks only, pricing information, estimated turnaround times, etc.

Q. Does Chart Access offer a paperless system for incoming and outgoing correspondence?

A. Yes.  Chart Access’ system is designed to offer a paperless option for the client as well as ourselves. Orders are received, processed and transmitted from our system without having to print any information.  Our clients can submit orders, check status and retrieve records directly from our secure, HIPAA compliant system.  More importantly, Chart Access can format images so they can be ripped directly into your system via File Transfer Protocol (FTP).

Q. How does Chart Access deal with special authorizations from a facility?

A. Chart Access offers the client choices when it comes to dealing with the patient and any special authorization requests.  We gladly contact the patient when necessary, but only when it has been mutually determined in advance by both the client and Chart Access.
